News about W. 4th clinic

To our patients:

Effective April 14, 2012 Khatsahlano will be consolidating all its Kitsilano medical services at the Broadway location.

Although the office at West 4th will close, you will be able to continue to receive care at the Broadway (2685 West Broadway) and Yaletown (920 Seymour) offices. Both clinics offer more extended hours: Broadway is open 7 days a week and the Yaletown clinic is open 6 days a week. Both offer evening hours.

What will happen to my chart?

Your medical records will be moved up to our Broadway clinic (2685 W. Broadway) after April 15.

Do I need to do anything at this point?

No. You are welcome to receive medical care from Khatsahlano at both of our other offices.  If, however, you wish to have your records forwarded to a different office,  please complete and send a release of records form from that office. By law we must receive a signed consent form in order to send your records.

My family and I need a family doctor; can you help?
